To join USMA click on the ADMIT ONE picture and print a copy of the USMA Membership form. Fill out the form and send it to to us at the address on the form along with your yearly membership dues.
Once your application is processed you'll receive in the mail;
- your membership certificate
- your DOT required arbitration process form
